Information for Patients
How Our Program Works
Loving Hands works with patients and caregivers, and their doctors, nurses, and medical advocates at hospitals, oncology centers and other treatment facilities. Medical specialists can refer patients for services based on their knowledge of patient’s situation and needs, or patients may request services for themselves. The process is simple.
Patient Information
When you arrive, we may ask you to complete an information form so we can learn about any sensitivities, special needs or requests you may have. You can download and complete this form ahead of time and bring it with you.
Contact Us to Schedule
After receiving their signed authorization form, patients contact us to schedule their chosen service. We do our best to accommodate your schedule. Services are performed at various locations, depending on provider and service requested.

Physician’s Authorization
We ask that patients provide a signed Authorization Form from their medical provider. This is to ensure that the services being requested are safe considering the unique treatments and circumstances of the patient.
